Lack of spatial awareness and the ability to use the map I guess. The difficulty of the water temple is incredibly overblown. Just go to the rooms you haven't been in and look at where the compass says chests are.
Part of me wonders if the amount of people who complained about the water temple dissuaded Nintendo from making more non-linear and actually challenging dungeons in future games, the thought of which rustles me a bit, because I like the water temple a lot.

MML2 actually has good controls, visuals, characters, cutscenes, gameplay, and humour though so it's not as soul-crushing to go through as the Water Temple.

Well, I would say it is an above average difficult puzzle and because this is a later level this is perfectly fine, and not too difficult. It is definitive not too hard as some people say. Zelda games always should have puzzles of this difficulty in later dungeons. You have to think only 2 or 3 steps ahead which is okay.

Every single time I watch someone play this dungeon they always miss the one key at the bottom of the central room and they get stuck forever.

In the 3DS remake, if you raise the water inside the central room, it shows a VERY long camera angle of exactly where the key is.

It's because it was hard when they were a kid 15-20 years ago. Typically a game that requires any level of thought as a kid is harder to complete than when you're an adult
I've experienced this recently with dark forces. I couldn't figure out the 3rd level (sewer planet) as a kid. I would always use LASKIP and go straight to the next level
Then when I picked it up around year and a half ago the 3rd level was easy and I felt retarded.

Because it was long and involved, required a lot of backtracking, the boots made it tedious, and there are two dirty tricks in the dungeon that can leave you stuck with no idea of how to progress. It is one of the few dungeons in all of Zelda that forces the player to think in three dimensions.

I know we have this thread all the time, but if you honestly can't see why a first-time player might find this dungeon hard, you're a little dense. Once you know the tricks (or if you're playing a newer version of the game that made the tricks less tricky) it's actually a very good dungeon.
