Yo! I've bought Tamagotchi Osutchi to Mesutchi
and I could use some help with the this menu. The cart has got a speaker thats supposed to make sound whenever the little fucker is hungry and such and I'm thinking this is somehow related to that function. Anyone got any clue?

You can select the speed of time, if you want to raise (your tamagotchi?) hurriedly, choosing fast might be convenient.

The first menu option is "Tamagotchi Time." The second is "Real Time." I assume this has something to do with the game's internal clock. The setting possibly alternates how quickly your Tamagotchi grows up or advances its needs.

OP, you probably want to replace the internal battery before you start playing. It's likely that an internal battery both keeps track of the time and keeps all your save data intact. So any Tamagotchi you create will be lost if the battery no longer has a charge.

If you can't solder in a new battery, you can actually pry the old one out while keeping the tabs intact. You can bend the tabs down over the new battery and apply a little masking tape to hold it. Always worked for me.

How is this game different from the original Tamagotchi game that came to the US?

I remember being disappointed I spent the money for a full price game to play what was basically the same as my much cheaper tamagotchi except it wasn't on all the time. The nicer graphics couldn't make up for the fact that I had to turn the game on whenever I wanted to check my tamagotchi.

I think the sequels keep your Tamagotchi running without the GB powered on, just like the little virtual pet units they're based on. My assumption is that the cartridge's internal sound would send out cries or other notifications, prompting you to turn your GB on and tend to your pet.

With the English release, the Tamagotchi was basically paused when the game was turned off.

Tamagotchi made no sense as a gameboy game unless it was a platformer with a virtual pet mechanic as a bonus to replicate the original hardware.

It's several different Tamagotchi species in one. The virtual pet units were released in series, with each series have specific themes or species you could raise.

The GB games also have a few extra features, like having a player character who explores a village and interacts with Tamagotchi shops, being able to breed Tamagotchi, etc.

My ultimate take away is that it was more pain than it was worth and I had more fun and better nostalgia from an actual G1 Tama on my keychain. It's pretty neat though as a novelty.

Nothing wrong with the pry-and-tape method. Once you pry the manufacturer's battery out, the tabs can be reused indefinitely. Future battery swaps are a lot easier and less expensive since you don't have to purchase slightly more expensive tabbed batteries.
