I'm about to play Hexen for the first time. Should I play with a source port, or is the vanilla game serviceable?

Source port, GZDoom works.

I recommend playing with zdoom, latest version can be found here. You just need the HEXEN.wad and to drop it in your Zdoom folder:

https://zdoom.org/downloads

GZDoom is just as good, but you need to disable a bunch of graphics options to make it look vanilla - otherwise you will have awful looking 'smoothed' sprites and the pixels won't be crisp.

just disable texture filtering

It is. I would not recommend using a source port for the vanilla wad. It will make it easier. The framerate cap and original mouse control are a part of the game, so no you're not getting the real experience if you play a more accessible version.

and sprite smoothing, and any bloom (not sure if they start on by default)

GZDoom has a little more to set up for vanilla than zdoom, which is why I suggested just going for zdoom. Not really any point playing with GZdoom unless you want dynamic lights which are not vanilla
