Neber. Get BloodGDX. It's an almost perfect port. No sound in cutscenes and room over room can look buggy at times, but that's it. Outside of those bugs it's a million times better than playing with Dosbox.

Holy hell, that boss is a nightmare on the two hardest difficulties. Went into the fight without much ammo and had to hit him at least 100 times with a pitchfork after using all the ammo I had and was scattered around the level.

I've beaten the game through Lightly Broiled (which was already hard enough to be the "normie" difficulty) with no cheats (with the exception of one level where there's a bug that a vital key is missing,
rendering it unbeatable
). What I remembered on the hardest difficulty is that you have to kill two of these flying shitheads with no voodoo dolls or tesla cannons. Hell, they could hover over you, kill your dumb ass with projectiles quickly, and teabag the shit outta your corpse.

Not sure what level you're referring to about the gargoyles, but AFAIK there's only two level where it's mandatory to kill them, and both only has one of them. Some levels have them as obstacles, but killing them doesn't really give you anything.

Video buffering and frame pacing (sometimes called stutter) make DosBox feel very different from a real DOS/Win98 PC or ports like BloodGDX. It's just not smooth at all times or feels off. A port is the best option and BloodGDX is really the only good one at this point (reverse engineered implementation of the 1.00 executable in java). I built a Dos PC just to play this game again and it's quite different (run 1600x1200 in DosBox; a Voodoo 3 can).

Anyone else run into this bug in BloodGDX where in E1M7 after you trigger the boss fight Cheogh begins spinning in place and ascends into the stratosphere far beyond any weapon's reach? Google seems to suggest otherwise

I've beaten the game with v1.11, where that bug usually happens. Rookie mistake, but I never knew that when I got this outta Emuparadise, which they had a large collection of abandoned DOS games.
