Did a lot of them. Though the various artist's they use now obviously took a lot of inspiration from him. Just google him and you're bound to recognize at least some of the movie posters he painted. He died last year sadly.

It was released during or after the show's second season. I'm guessing Konami just choose the coolest stock art Image Studios had available, since it was also used on the JP flyer for the first arcade game.

As someone who owns a few boxed SFC games, they're roughly the same dimensions but a bit longer. Usually SFC boxes have the packaging artwork arranged vertically, but some of them (like the Final Fantasy series) have them horizontally. It varies between games.
