The fingers and overall look of the face. And there are no lizard enemies in RE2 either like the Hunters in RE1. It's got Lickers instead in their place and they not only not have eyes they have claws instead of fingers.

The RE2 cover is a typical B horror movie cover. The idea is to give you a general feel without showing you the whole actual monster.

The "door-peeking" is just to remind RE1 fans of that scare you got when there were monsters immediately after the door-opening/room loading animation.

Honestly I'll chalk this up to artistic liberties and things possibly changing during development.
Might not be 100% accurate to an ingame situation but the cover gives you a good idea about what kind of game you're looking at. Streets of Rage shows a character fighting punks and thugs on a street, not entiry like it happens ingame, but it gives you an idea

It's like the cover to Doom, there isn't a gun which looks like this, demons can't actually grab you or climb ledges, and the only demon with a gun arm is gigantic and wouldn't fit in this image, but it's dramatization, it shows the hero in a heated battle with the spawns of hell, it also hints at co-op multiplayer in the background. It's a dramatic image meant to convey to you what sort of game it is.

The Europeon cover is infinitely better, it looks like a classic movie poster and characters are correctly depicted.

I don't think anyone was shocked or offended at Phalanx, either they saw it and maybe dismissed it as something that didn't interest them, or they picked it up, read the box and wondered what the hell is with the cover.

Or if you actually were familiar with the game, it might be even more confusing because you know exactly what the game is and the cover is mystery to you.

I haven't played Phalanx but as I understand it's some kind of space shooter, did the devs ever comment on the choice of cover art?

I really like that box art. It's better than the US version. It's simple and only uses three colors, but that's what makes it good. Biohazard 2 (Jap) has the same box art as Euro Resident Evil 2, by the way. Both box arts aren't very representative of the game though, one had lizard guy, the other had skeleton man.

It's interesting how US covers tend to have 3D renders and guns while EU and JP covers are generally more simplistic and feature drawings and abstract things.

Anyway, a lot of covers are pretty in this thread are pretty representive of the game. It might be bad art or not exactly as shown on the cover, but you definitely get the idea from most of these covers. Mega Man is a guy who shoots, Chrono Trigger a medieval fantasy adventure, Resident Evil horror game and Streets of Rage a beat'em up.

I've seen all the alpha and beta material released and leaked, there really isn't anything that quite matches it in looks.

There's a few pickup sprites with a shape vaguely like it but there isn't anything from the first person perspective that matches the rounded off boxy shape.

How? The Euro covers show an abstract/stylized depiction of Dr. Salvador, one of the notable enemies in the game.

I guess maybe it gives the impression that it's more horror than it actually is, but I don't think it's THAT egregious, the game is still aiming for a horror-like atmosphere.

I think it's a much more visually striking and interesting cover than just photoshopping in an array of characters in that typical cheap modern day Hollywood arrangement.

There are many misleading things about Mega Man 2's boxart. Mega Man doesn't wear a motorcycle helmet, there is no orb that looks like the Death Star. Mega Man doesn's use a phaser and Dr. Light doesn't fondle Crash Man's shiny metal ass. Their faces are also off. The only thing it does right is that you shoot robots.
