Besides the obvious (Cho Aniki), I think the gayest video game is probably Persona 2.

Phantasy Star 2 had the first overtly gay character in a major console game.
Ustvestia (I think that was his name) was this NPC character in some town or other than teaches piano. There were a handful of games prior where a character was implied to be non-hetero or where the character's preferences or gender are left heavily ambiguous, but PS2 had the first absolutely incontrovertibly gay or bi character.
During the events of PS2, the party needs to learn piano (the MUZAK technique) in order to do a thingy somewhere sometime in some dungeon at some point. The player can choose which party member learns piano. If the player chooses a female party member, Ustvestia charges full price.
If the player choose a male party member, he says "Oh, he's cute!" or something like that and cuts the price in half.

Persona 2 had the first bisexual protagonist in console vidya, I think though Curtis, the MC in Phantasmagoria 2, was heavily implied to be gay though not stated outright. His best friend, Trevor, though, was gay and Curtis held feelings for him in a very homo-gay sorta manner.
Phantasmagoria 2 was a terrible game, gaiety aside.
