I'm a survival horror fanatic and there are two PSX jap only survival horror I've discovered a few years ago.

>Devilman
First half of the game is survival horror and all you can do is run away from big monsters. You can also hide and use items to block the path à la Silent Hill Shattered Memories (except years before it). It's overall rather basic but it works and is worth checking out for its rather 'original' take on survival horror (I know clock tower did it before but it's still pretty different here)

>...Iru!
A first person survival horror that is a mix between Clock Tower and Echo Night. You'll need to understand japanese to fully understand the story. One puzzle is also really hard to figure out without moonspeaking.

God the Famicom had so many, I don't know where to even get started. I guess off the top of my head:

Konami Wai Wai World 2 - Simultaneous two player Konami mash-up action! The music is sex.

Cocoron - Platform game with customizable characters, made by the original Mega Man team who later went on to make Little Samson.

Goonies - Technically not Japan only, as it appeared in arcade cabinets, but never released on the NEs, strangely enough. Good adventure platformer with 8-bit Cyndi Lauper!

Getsuu Fuuma Den - Amazing mashup of Castlevania and Zelda II gameplay, worth checking out (it's Konami)

Ganbare Goemon 2 - Not the first Goemon game on the Famicom, but this is where the fun really begins.

Layla - Stupidly hard Metroid like action adventure game with a similar sci-fi setting and female protagonist.

Yeah that game is really good. The TMNT game on NES by konami was based on the engine of this game so it's like TMNT but better.
The only thing that suck in that game are the "3D" mazes with the camera behind the character's back, those are terrible.

On Famicom I love:

SHMUP by Konami, just a shame the 2p mode lacks and flickers so much

If you have to play ONE shmup on famicom play this one, one of the most technically impressive game on the system

>Moai Kun
Konami again with a puzzle platformer. You have to push rocks to save your friends on each screen while making sure not to block yourself and while avoiding enemies. The game Toki Tori was obviously heavily inspired by this game.

>Splatterhouse Wanpaku Graffiti
Splatterhouse SD style. Not as challenging as the other games in the series, but still challenging enough. A lot of humour in an SD style too.

>Kyattou Ninden Teyandē
based on that TV anime with samurai cats. It looks like your basic platformer but what makes it fun is that you can switch at any time between various characters all with different powers.

>Spartan X 2
A very short but very fun beat'em up. The kind of beat em ups that feels oldschool because you only play on a straightline.

>Moon Crystal
A platformer that's just challenging enough with beautiful graphics, great characters animations, moody musics and a story told Ninja Gaiden style.

>Chōjin Sentai Jetman
A short and easy action platformer that feels like it uses the engine of Shatterhand (by the same developers too)

Yeah ...Iru! was re-released in some "best" release for games that sold well, kinda like Platinum.

A friend of mine started writing a translation but the links are dead now.

I started writing a walkthrough to get 2 of the endings (the only 2 I could find) and the walkthrough is about 95% finished for those 2 but I never got to finish it.

I really love that game too, it and Umihara are definitely in my top five Snes games. As for Japan only games I like it's hard to think of some not already mentioned here. Though I gotta say Famicom Mystery Club, Kunio Tachi no Banka, and Twin Bee Rainbow Bell Adventure. (I think it was Jap only at least, it might have been Pal too.)

Captain Tsubasa games can be played with little to no japanese knowledge. The fourth one is the only one that doesnt have a translation patch. Also, if you can read katakana and hiragana you can read everything, it doesnt have kanjis.

Battle Pinball - Best 2d Pinball ever. Jaki Crush (sequel to Devil's and Alien Crush) is also really good. It has a dark/evil Japanese mythological theme that's really cool.

The PSP version was broken trash and it's not like you need to know any semblance of Japanese to play Umihara. At least Sayonara is being localized though.
