Zelda is transformed magically into a boy. The OoT Manga clearly states this, and Aunouma has stated that elements of the Akira Himekawa mangas are canon unless the games outright contradict them.

Shiek's gender is referred to only once; Ruto refers to Shiek as a "he" in the Water Temple. However, that could just be because Zelda disguised herself very well as a man, not that she magically turned herself into a man.

You can clearly see the wrap holding her breasts down. There's no reason to imagine she actually physically turned into a boy unless you want to believe that for whatever reason she also didn't/couldn't transform her breasts - or her face, for that matter since it also had to be wrapped.
