Outside of just the art even, I guess.
The atmosphere, the straight cultural horror and shit that people really believe in really makes it stand out and so much more disturbing, even for a shitty point and click adventure game? It really puts you in that period of shitty Japan where plagues are everywhere, life sucks and crazy people are everywhere while being trapped in this hierarchical hell.

Not a 100% fit, but you may enjoy Juggernaut. It doesn't keep the Japanese mythology theme but it's an adventure game with a very surreal tone and atmosphere, I've never played anything quite as weird as it.
