There were graphics hacks of Super Mario Bros. back during the dawn of emulation. I remember one that had you playing as a UPS driver based on the UPS strike of 1997.

No, this isn't a romhack, it's a seperately developed piece of software.
any of the 6 million chink FDS versions of Super Mario Bros would be the "first romhack," or Atari stuff that was floating around BBSes in the early 80s (research "Glenn the 5200 man")
