It was simply too ambitious. Making it so that every episode has completely different enemies, level design, and weapons is cool in theory, but it doesn't work well in execution. There are good elements of each episode that if put together would have probably resulted in a great game, but as a whole it's a very messy experience.

Didn't have anyone to check him on his unrealistic ideas, didn't have one of the most proficient software developers of all time to tackle his cutting edge ideas, naturally confident but had a boosted ego from Doom's success, copied his own tactic of challenging the player base before the game even came out(but didn't hold up to the hype like Doom E1), a bunch of other tricky tacky things. At id he was part of a team, at Ion Storm he was the boss. Early Id was lightening in a bottle, most of those guys never saw that level of praise again but it seemed like Romero was more intent on recreating the success than recreating the situation that made him successful. It honestly wouldn't be even that much terrible if the first enemies you meet weren't robotic dragonflies / frogs / crocodiles - and a random unkillable remote turret that can pretty much instakill you if you don't find and destroy its power-pack in time (that is in those few times the level design requires you to do it to get past them - otherwise they're just "border partol" that kills you for going the wrong way)

IIRC she mostly contributed to Episode 2, which ironically enough is one of the better parts of the game.
