Can anyone explain to me why this game is the goat? (I also tried arcade)

It's not, and Saturn isn't even the best console for shmups imo. Even then there are better shmups on it like Sengoku Blade and Hyper Duel. I haven't played any shmups in a while but I had way more fun with SEGA CD and TurboGrafx CD when it comes to shmups. I did play Detana Twinbee on Saturn a month or two ago and that was pretty fun.

I hate playing Garegga myself but I honestly do see the appeal of it. Most shmups at the time made dying HUGELY detrimental to the player, to the point where 'Gradius syndrome' is something everyone understands the meaning of immediately. Garegga on the other hand, makes scoring/dying into a crazy risk/reward system that can be pretty satisfying to pull off.

It just gradually grew on me. I didn't play it exclusively at first, just now and then but kept coming back to it. It's just such a complex game. And the more I play it the more I realize there's new things to try, with the rank, with scoring and different weapon options and secrets to discover. There's just so much. It feels like there isn't really an obvious way to play the game optimally. Maybe there is one but for me it's enough that it doesn't feel like there is.

Just play Batsugan and you can see the DNA of a typical bullet hell form before your eyes.
It's halfway between a typical Toaplan shooter and what would become Cave when the staff members left Toaplan to form it.

there's no real "first one" because there's no arbitrary limit that makes something a bullet hell or not. Stuff like DDP is considered by everybody to be a bullet hell, stuff like Tiger Heli definitely isn't, but Batsugun and Garegga are in a weird in-between, with some emphasis on micro-dodging from time to time, but with a fairly big hitbox, low bullet count and density, which makes them feel like prototypes for bullet hell.
Batsugun isn't even too different from V-V when it comes to bullets, and that came out a few months before, so that would make V-V the first one.

It has a super autistic rank system that is way overly complicated and very artificial. I guess most people like it for that and the scoring (which is also a pain to optimize due to rank). Personally the rank system is a negative for me and the gameplay is only decent.

Tiger-Heli has more bullet curtains than most classic shmups to be honest, but I guess you are talking about patterns from a single enemy.

I think the main defining factor is small hitbox, which is intrinsic to all the gameplay and design surrounding it. To me, danmaku's signature style is a tiny little hitbox, slow bullets with tiny hitboxes, dull and static backgrounds, a shot covering a large portion of the screen (at least 50%), uninspired art and sound, and constant hordes of enemies littered without intent or care that just form a long flowing chain.

I would say Batusugun Original is only like 40% danmaku, and Batsugun Special is maybe 60%

Thunder Force is different than Garegga in a lot of ways. Obviously Thunder Force is mostly hori while Garegga is a verti, but beyond that the games are from two very different eras. Thunder Force is a survival game, the point is to defeat the final boss. Garegga is more modern in that its based around high score and has complex scoring mechanics.
