Here's one that seems very silly, but hear me out. When I was a kid we had our NES hooked up to an old TV that my parents got in the 80s from a hotel that was throwing out their sets to replace them with newer ones. It was very fuzzy and low-quality. Because of how fuzzy it was, the FF1 Black Belt's fist looked like one solid flesh color that matched his face color and the blue line between his face and fist was not visible. On top of that, when he punched the fist in front of his face didn't move (or not that I remember), so it looked like a hand was coming out from the other side to punch.

tl;dr - I thought the FF1 Black Belt's fist was a big round clown nose because of our awful old TV.

You want a real explanation? It's probably a combo of just wanting a simple, cartoonish art style (very reminiscent of games like Pac-Land, desu) and also cartridge space limitations (big, simple flat shaded graphics are a lot easier to tile and compress compared to something more shaded/detailed, and cartridge ROM was still pretty expensive at the launch of the SNES). One of the Nintendo leaks a couple years ago included multiple iterations of Mario's player sprite for SMW in different styles, so it seems pretty likely that Nintendo experimented with the game's visuals before settling with the style in the final release.

Chrono Trigger is notorious for this. The sprites are so detailed and so many enemies are weird alien things that it's easy to see things wrong.

Almost everything about it is tailored to cause confusion. It's an evolution of the Ocean Palace despite looking totally different. And the word "palace" implies it's a stationary building rather than a mobile submarine so when it starts floating in the sky it's easy to assume the same thing applies. You do fight her on the "top" but what counts as the top is really the center of the sprite. It's the top of the ship's bridge. And you really don't travel vertically all that much. The Epoch drops you off in the back by the engines and you spend a lot of the time walking north. You only take two elevators in the entire thing.

The Black Omen's floor plan is actually very accurate to it's intended design. It's just the overworld sprite that's a mess.

I don’t know if there’s a name for this phenomenon, but another one which is often involved in the creative process is the artist going through multiple drafts/redraws and not realizing that the one they settle on doesn’t convey the information they were originally trying to communicate.
Basically, if you went back in time to the artist who was responsible for drawing Bowser and showed him that sprite before he’d started working on it, he’d probably ask why the eyebrows were so weird and had a part which looked like pupils; but if you let him go through all of the iterations he did where he was trying to get the sprite just right, he’s going through a thinking process the rest of us aren’t privy to, to the point where he probably couldn’t see it the way OP does if he tried—his brain has different and more information about what the sprite is supposed to be than we do, but at the expense of him not seeing flaws which make it convey something different or incomplete to the audience. (I could also see that being a factor in the star destroyer case, even though that was also likely muddled by having different people work on objects and backgrounds).

This is why it’s important to re-read essays before you submit them, and get the perspectives of others from time to time when drawing—you might have started out with a clear vision in mind, but along the way edit out some lines where you justify one of your conclusions and make it look like you just pulled it out of your ass at the last second; or you draw two or more overlapping details too close together in a way which makes your drawing look like…well, some of the sprites being discussed ITT.

It's more likely the sprites were based on earlier models and they redid the promo art later on. That's why the original SNES sprites for Tales of Phantasia look nothing like the character art. The designs were revised later on and it was too late to change all the sprites so they just ran with it, eventually updating everything in the remakes.

Another one would be the brown bricks in the background of stage 5'. The entire section is just that one brown brick repeated all over the background. Then, if you check the beta version of the version, that same brown brick used to be all over stage 3 before they detailed the place. In other words, the detailing was done in stage 3 but not 5'. Most of 5' is pretty bad, being just a series of non connected segments re-using backgrounds/areas from other levels, and it doesn't even have a boss.

Another example would be 2', at the very start of the stage there is a secret exit on the bottom left side which leads.... to the exact same place as the alt exit at the bottom end of the stage. Thus rendering the entire stage pointless. That 3rd exit only makes sense if it was to lead somewhere else.

Seiken Densetsu: The Emergence of Excalibur was in development much earlier. We're talking 1987. It was going to be a massive Disk System game that would've spanned five disks. When it got canceled, Square asked people to keep their preorders for Final Fantasy instead. It's generally believed that it has nothing in common with the eventual Seiken Densetsu / Mana except for the trademark. Secret of Mana uses a version of the battle system that was planned for Final Fantasy IV* that was rejected in favor of the active turned-based option. Chrono Trigger was known for supposedly taking unused ideas from the CD version of Secret of Mana (itself having the workimg title "Chrono Trigger") and the SNES version of Final Fantasy VII. That may be party why Crono resembles Randy.

*I'm still unsure if this was planned for the reeased SNES Final Fantasy IV or the canceled NES Final Fantasy IV, which were different games (IV was originally "V" but was set to release before IV anyway so NES IV was doomed from the start).
