Not retro but pic related is infamous for its game breaking bug. If you create a save file with a name of three characters or less the game will just brick the game entirely. They tried fixing it for later printings but ultimately had to end up just packaging the game with a warning about it.

While not retro it's a pretty old game. Lord of the Rings War in the North was a fantastic game but there is a game breaking glitch at the end where trolls are supposed to come out, the game saves and then trolls never show up. You can't progress because the enemies never come even after quitting/loading. I'm not sure what triggers it though as I have friends who say they've beaten it and I have friends who say they ran into the same issue I did.

Two player mode is unbeatable since it will never let you win past tag the monkey. If you contacted Acclaim back in the day you could trade in the black cartridge with the bug for a grey cart with the bug fix. Very few people did this and the grey cart version is rare and worth hundreds. I was lucky enough to swap my game with Acclaim for the grey one and held onto it.

If we're counting not being able to 100% a game, Whiplash's completion counter stops at like 95% or something because they just straight up calculated it wrong. You can do everything in the game, but your save file will never actually say 100%.

There's a bug in Metroid Prime 2, near the end of the game right up in that neat techno area, that if you save the game with the bug triggered you lock yourself out the area and you need to AR the game to get by it.

Everyone talks about GT2's max speed garage bug, but a lesser known bug exists in GT4 in which you can be locked out of 100% completion if you do not do the first driving mission before any other driving missions.

For the German release of I Have No Mouth Annd I Must Scream Nimdok, his entire scenario, and anything related to him was removed due to the large role Nazis play in it. Beyonf being censorship this unfortunately resulted in the game not being beatable, it was never properly tested and still requires you to complete his scenario despite it not existing in this version.

Not as game breaking as a lot of the other games posted here. But I've got a serious vendetta towards pic related for absolutely cucking my younger self by freezing every time after you beat the final boss for disc 2. I couldn't figure out what was going on and just assumed my disc was busted and ended up putting it down despite getting into it. Years later I decide to try again from the top and bought another disc from online only for the same thing to happen. I finally wised up and looked into it, and this whole time it turns out for whatever reason the game shits the bed if you use the dragoon transformations here, and beating it without relying on those keeps the freeze from happening. I was pretty annoyed to discover this whole time I could have kept playing if I had only known about this retarded fact. From what I saw nobody really knows why it happens. The most that could be said is that its apparently not any issue when played on a ps1, which I had long ago dumped in favor of just using a ps2.

I don't know if someone suffered this, but during my playthrough of Gun on PS2 years ago, I decided to save and continue later on some part of this mission:

On it you're weaponless for a while and then you recover your weapons. I think I've saved after that, and when I reloaded my save, I lost all my weapons and only could kill enemies with the tomahawk (weapons are obtained while advancing the story and aren't like dropped from enemies). Since you have to shoot the boss to kill him, it rendered my game unwinnable and lost a lot of progress. Got really mad at that since it was a fun game and didn't play it again until last month.
