The sprite doesn't have proper definition for her nose, so I always assumed it was some skull with no nose and the hat covering the eyes or with no eyes. Her head moves from side to side and acts as a symbol for a timed challenge. I always thought it was supposed to be a skull in a witch hat counting or chewing (the timer sound sounds like chewing.)
