Touhou Project games are popular for the characters and the music, not the gameplay, which is inferior to good arcade shmups.

there are more too many maids in the PC-98 games, but you can easily avoid seeing those characters by focusing on the windows era

People like it because it has great aesthetics and music. Superplayers love it because it's extremely hard and the chain-based scoring system is very punishing (if you drop your chain your score is basically fucked, so you need to play perfectly). It means you need to have perfect routing and strategy on the levels and execute it perfectly to get a good score. So there's almost no room for fuck ups if you want to play for score, even more than other games. The second loop also wipes all your lives (in white label, black label changed this) making it extremely fucking punishing because you can't die once in the second loop before stage 5 or so or your run is ruined. But that's a minority of a minority, most people just like it because it
