You know what's weird? There are a handful of unused levels in SMB3, and most of them are complete enough to be beaten (all they really need is to be fleshed out with more enemies and obstacles), but when it came to making the e-reader levels they didn't use any of them. I mean heck, they could've taken the opportunity to finally put the infamous para-beetle level from the back of the NES box in the game, but instead they went with all new stuff that didn't originate in 87/88.

Well including new content in old games at all is out of character for Nintendo. I would just like to see them acknowledge the half-finished stuff they leave behind for once, like the medallion powers and sword beam in OoT, or the unused levels and bonus games in this game (pic related). The answer is much more mundane though.

A lot of SMB3's GBA port was based on the GBA engine for Super Mario World, but with SMB3's moveset. It's missing the randomized sky icons from the NES version of SMB3, and the unused levels themselves are still in the GBA data but never converted properly because the team didn't know what to make of them, but couldn't just delete them either (lots of unused levels share the same maps as used rooms).

They decided it's much better to add new content as a separate mode, and were experimenting with DLC as a new distribution method (Fire Emblem Fuiin no Tsurugi and Mario vs DK had downloadable levels too but only through save editors at promotional events, nothing like the e-Reader)
It wasn't from the goodness of their hearts.

Nintendo is as bad reusing old leftovers as Capcom.

Depends on the rom. If you're using the Wii-U version which is rev 1 and the one everyone should have it's all baked into the rom and unlocked from the start. With the original GBA release the cards need to be scanned and everything is stored in the save.

USA is best experienced with the annoyingness intact. They parsed that game three times to try to make it fit, first they added mario sprites and a run button, second they updated the graphics for the SNES and somewhat made it fit more with the rest (may be a stretch), third they added in pointless yoshi coins and experimental-tier bad dialogue, also giving it finally a universal title, it really is the definitive version of that strange little game.

Four times.
Third one was BS Super Mario USA on the SNES Satellaview which added three collectible Yoshi Icons per level.
The Advance port has only two per level (now Yoshi eggs), some inspired by the BS version yet have clearly more meaningful placements (in BS you'd often find them out in the open, some almost unavoidable where you fall down)

Advance's main additions were some extra gimmicks and items, and replacing one of the recurrent bosses with a unique one (last iteration that did this was the first SMB2 NES version that replaced the final white mouser rematch with a unique boss), and that infamous weird sprite.

For the Wii U version (I recommend the European Wii U version) all the levels are loaded, but e-Reader functionality still works and is still needed in fact to access the main game's side of e-Reader additions (gameplay modifiers, developer ghosts, extra items), and you can still scan the original e-Reader cards just fine (just not save them)
Sadly, they broke the sleep feature somehow and it just crashes the game now.

Collecting e-Cards for the US version is just asking for a miserable experience.
The US set was never released in full: Nintendo translated and adjusted all 38 levels for an English release but they sat in their vault until the Wii U. So there's 28 levels and other modifier cards in some collector's basement from discarded NoA materials.
You want something even better? Rumors about the European e-Reader are true. It does exist. SMA4 PAL will unlock the e-World menu if it detects it, and of course it will have e-Cards regionally flagged to Europe.

Either collect the Japanese set and game, or just emulate it using Nintendo's official complete Wii U version.
With the Wii U version, only e-Cards needed are the non-level ones. (Keep in mind the 100 Up card is only usable once per save file)

There's a glitch in Sonic Advance 1 where if you reset your game your collected rings won't go to the chao garden anymore. You can fix it by starting a new game in an emulator and upload it to your cart.

You can also upload games to an e-reader without the cards.
