It is fan fiction from an official source. The endings are happy as it is written as if it slots right into the TV series. It isn't cannon, but they sure give Mana all the appeal worth remembering.

Iron Maiden is not "canon" because it wasn't written by Anno, but it still had a lot of TV series staff working on the art, and the guys who designed the game were veterans at Gainax doing games and software, it wasn't outsourced, it was made by the in-house Gainax software devs.
The story in Iron Maiden can perfectly fit in the action arc (after Asuka arrives, before Touij becomes a pilot), there's nothing in the game that contradicts canon

Well, there's this scene in the game.
It's censored in the PS1 version, but not on the original PC or the Saturn version.
And yes there's a scene with Mana naked (you can see tiddies), but I'm not sure if that was on the console versions, likely not.

I've no idea, the version of Girlfriend of Steel that I played was from a rip of the DVD-ROM version from the second impact box from 2001, so it wasn't the original CD-ROM version, but I'm sure nothing was changed. Can't speak for the later re-release for PC or PS2.

He has already admitted on the booklet of the Khara 10th Anniversary Expo celebrated on 2016 that one of the reasons why he wants to Gundamize Evangelion, ie, to let people do his own takes on Evangelion at Khara is so they can have a nice source of money that can be reinvested on new projects because nothing can be done without money.
Meanwhile with Yamaga and fellows guys at Gainax you have them being a bunch of utter incompetent idiots when it comes to care of their own company, plus the villany of they backstabbing and backtracking on closed to finished deals with Anno.

3.0 was weird but so was episodes 25 and 26 of TV series, and Death and Rebirth (which only contained 25 minutes of new footage and ended on cliff hanger). That made fans of Eva in 1996/1997 go mad to the point of sending death threats to Anno, but you already knew that.
Anno is going to do it again with 1.0+3.0, prepare for the next EoE, and prepare for the internet exploding and imploding at the same time. Never doubt Master Anno.
