It's just a kinda slow beat'em up with surreal environments and enemies and some unintendedly funny moments. Powering up your guy into a beastman with super powers was pretty fun.
Let's just say that after Sonic became the Mega Drive's pack in game it started to sell a lot more.

It’s an arcade game, it was always meant as a 10 minute quarter ride. When they brought it to the genesis, it really is just that, you play it for 10 mins and put it away.

This, combined with the level-select feature in the same cheat menu, is what let me practice enough as a kid with the Genesis version to be able to consistently beat the game now with normal settings. actually "easy" settings because I always give myself the most lives/health in the regular options menu before I start

Dragon was objectively the best beast form, in terms of usefulness ("coolness" subjective). Completely OP.
>Only form that can move at faster-than-walking speed in all 8 2D directions at will
>Has completely unique and cool standing sprite if/when you decide to touch the ground and stay still
>Can use a rapid-fire max-dmg surround-attack with functionally instant startup, instant cooldown, and no forced movement, which can easily wipe any boss in under 30 seconds, without the use of a "turbo"-equipped controller (bonus points for ear-bleeding sound effects)
>Has a cool lightning-bolt projectile on top of everything just because there's still another attack button to use
>Comes in vivid deep green (1p) and fucking gold or shit-brown if you wanna be a dick (2p)

Cave is notoriously fucked. If it weren't enough with the sheer number of insta-kill bottomless holes, the sanic ants that will drop in and bumrush you into said holes, the surprise (to the unaware) tortoises that will both cause knockback and turn you to stone, just so that even if you don't fall into the aforementioned holes while trying to mash your way out of stone, it then sets you up for those boxing demons to haymaker your ass into the holes anyways, they throw in a chicken-lizard near the end, just to remind you that there is no forwards low kick.
Easily the hardest level, Caves was notoriously designed to DEVOUR quarters for the arcade version.

It has pits you can die in. It's a platformer.

And yes I like Altered Beast. It doesn't really have any flaws. Great sound, graphics, controls, story, pacing and the difficulty is just right. Only people who hate it are the ones who can't even beat it.

I think it was secretly a great pack-in game.
It's spectacular and short (and at least a little difficult) so you can be done with it quickly and have a ready appetite for more games.
