1) Buggy as fuck on anything non-vanilla
2) Performance is ass due to how it does what it does (spawning and despawning 2 actors for every monster on the map 10-15 times per second, among other thing) Overall spaghetty code

Brutal Doom is cheap fun for a little while, but it's too bloated, so it gets annoying if you're not autistic. The best part is how the gameplay is ramped up, and the zombiemen are actually a threat at times. The worst part is how imbalanced the gameplay is because the weapons are too overpowered.

The Brutal Doom community is such a circlejerk, that it annoys nearly everyone else that likes Doom, especially other modders.

SgtMarkIV is a prick. He hates on good games, and praises his mod in the same sentence, like he's the guy that invented Doom or some shit. His brainless hivemind of teens praise his every move, like comparing the charred corpse of a zombie to black people.

Then, there's the fact that there are several gameplay mods that achieve the same good things, while not screwing up the core of what makes Doom fun. Namely:

The only reason I play BD is for its realism mode.

Some of the most adrenaline pumping moments I've ever had in Doom were from playing relatively easy wads (e.g., Diabolus Ex) run on hard realism difficulty.

It's exactly how I wanted Hideous Destructor to play.

You're not particularly missing out. It's hardly impressive with its gameplay, and the flashy detail and gimmicks have long since been outdone.
If you wanted to play a mod which turns the gameplay on its head (and which is hard), there's Hideous Destructor, if you something that's still a fair bit like the base gameplay, you have stuff like Burl Tumd.
