I think there isn't a real Secret of Monkey Island, perhaps not even Gilbert himself knows it, or maybe he would've thought about it after the first and second chapter but that's unlikely because that name was already used, rather it's a plot device just like Pulp Fiction's suitcase. Although at the end of MI2 something was indeed boiling up, that doesn't have anything to do with the "secret" for plenty of different reasons. Overall, it's just a nice title that plays with the mind, making it sell more copies.
