I somehow can't recall anything really "game-y" about it. I just remember playing this like a terrible adventure-game puzzle, just hitting everything.

I'd say Carmageddon is more fucked up since you kill way more people in it and it's way more fun.

that game is incredibly awesome for the year it was released.

Not sure if retro but soldier of fortune 1 was insane upon release.

I remember blasting dudes in the first level on the subway with the shotgun. Some limbs would fly off, other guys would scream as their intestines would pour out.

Time Killers - found this in an arcade at age 9-ish, imagine my shock when a character cut my fucking arm off mid-fight, and the round continued while the arm was still twitching on the ground.

Dreamweb was also a very nice game not only in terms of violence, but this was a rather atmospheric game. IMO this also was one of the inspirations for Hotline Miami, despite the games being completely different gameplaywise, but the atmosphere and aesthetic is very similar.

I have to agree that all things considered the Horrorsoft line is probably the most "fucked up". It covers all the bases with immersion, a powerful sense of dread and graphic violence including sexually suggestive elements. I'm glad that I didn't encounter these games when they were new as they definitely could have scarred me at that age.
