D, Enemy Zero, and D2 are pretty cool and underrated.

Also not retro technically but one pretty interesting game is Nanashi no Game or "Nameless Game". It's about a guy who finds a haunted retro game that kills anyone that plays it in seven days, pretty cool as you experience horror in both the actual game and the retro game the main character found.

One would think Sega marketing the Genesis as the "mature" console there would be more spooky games but aside from Splatterhouse 2&3 mentioned above, maybe Ghouls n Ghosts I guess. I can't think of any aside from this.

Seconding Clock Tower: The First Fear (SFC, there is a great translation patch) and Clock Tower (PSX) as supreme when it comes to spooky games. You will not regret checking them out.
I own and have repeatedly played each game in the series, but the first two (mentioned above) are really the only ones I'd recommend.
Clock Tower: Ghost Head (PSX) was a departure from the story and characters of the first two games and was, in my opinion, a CT game in name only. I wanted to like it so badly, but it was a failure with an interesting concept. It's worth playing because it is indeed a horror game with a neat first scenario, but it feels unfinished and it very frustrating/boring as it progresses.

I'd also like to recommend another severely underrated series of games: the Echo Night games.
Echo Night (PSX) is a fascinating and horror-steeped game that is as fun as it is challenging to work through.
Echo Night 2 (PSX, a flawless translation patch is available) was, unfortunately, only released in Japan, but is absolutely worth downloding and patching. It is superior to its predecissor in every way: a more engaging story line, multiple endings, considerably toned up on the horror, and some of the most enchanting graphics I've seen on the Playstation. I cannot recommend this game enough. I was captivated by it when I played through it last summer. Please do yourself a favor and check it out. I played through it before EN1, and I don't regret it but it was certainly the superior experience and EN1 was slightly disappointing as a follow up but still a great game.

I agree with most of this list, but some of it's not so bad. Even Nightmare on Elm Street is at least tolerable, though there are unsettling pieces in it.
Most of the Sunsoft games on there too, they're not so rattling.

Xenophobe is less freaky in the arcade, so I'm left to believe it was intended to be comical. Late 80s/early 90s Atari did come off that way.

Elvira 2 for the Amiga. Currently playing this game and it requires lots of patience as well as trial and error. Took me quite a while to get used to the combat, interraction and pretty much everything but the manual really helps. The game iself feels like you’re walking through a haunted house that wont hold back with killing you. Should you play this game you will die many times - don’t get me started on the game over screens. The music is one of, if not the best for the amiga. Oh yeah this game is very hard to beat without a walkthrough but solving problems is very satisfying. I hear ‘Waxworks’ is this game’s spiritual successor but I’ve never tried it.
