They did not. That is not a screenshot, it is a mockup. It's not even the actual fire mario sprite, m8. I doubt the person who put that together even played the game, they just looked at some sprites and roughly recreated as much of what they saw as they could without thought of context.

The wall is also correct but a different colour. How could he not have noticed Mario was directly above the fire? That's exactly what I'm saying. The modified colours, probably breaking the NES colour pallette rules, show that it's a recreation. It's entirely possible that in 1985 when this cover was done the artist had not seen enoughh video games to even realise that the red stuff is supposed to be lava.

They were making videogames since the early 1970s and Game and Watch was an outstanding success and it was released in 1980. Now they were doing something absolutely huge and they had incredibly talented people working there and it was for the launch game on their console.
