On those models the AV output is mono to keep backwards compatibility with the SMS cable. Using the frontal HP jack, you could get actual stereo.

Because in Japan, it was standard at the time to sit 2 feet away from your TV with the console right in front of you. Japanese also typically live in smaller homes, so having a TV blaring bings and wahoos would be disruptive to everyone else, hence the headphone jack.

Why you're only able to get stereo audio out of the headphone jack will always be a mystery to me.

A better question is why do you have to plug into the headphone jack instead of the back of the console if you want stereo sound? That's right, only the front audio output is stereo, the back is mono only.

Not really a problem since I use separate audio and video cables anyway, but really weird.

My TV is 5 feet away from me. But Sega Genesis is only 3 feet away from me. I use my auxiliary cord to connect my surround sound system to my Sega Genesis. If I was to use headphones, I would position the Sega Genesis to another spot closer to me, so that my headphones could be plugged in.

It more had to do with the apartment type setting. Where the TV might be in a room connected to other rooms. So you can listen to the game and the audio doesnt disturb mom in the kitchen or dad at his table. It was basically like being at a lan party.
