For the Sega Genesis it's almost certainly the cartridge version of Eternal Champions.

Forget beating the game, you'll be lucky to make it through the first fight until you've read up on moves and combos.

Nemesis '90 Kai may be the most hardcore horizontal shmup I've ever played. Otherwise, this platform is full of shmups and ports of other difficult arcade titles.

Vagrant Story, at least on your first playthrough if you aren't using a strategy guide. Alternatively, King's Field 2 or 3.

I would say the most difficult retro game ever made would be between Ghosts 'n Goblins and any number of shoot-'em-ups.

Road Rash 2, the game is impossible to beat because it has no ending. After you beat the last few levels (I think it was the 5th level), you end up warped to levels which only have cops, and if you beat any single course, the game crashes.

Speaking of which, the MAME team added a new recompiler for CAVE SH3 games, they no run twice as fast.
And they just recently dumped Akai Katana, but it's not officially supported on MAME yet because they couldn't get blitter delay (ie slowdown) to work properly so you're need to find a custom build for that. Pretty cool, huh

Radiant Silvergun Saturn mode. Takes about 90 minutes to clear and has a shit ton of boss fights, plus the game forces you to score lest your ship stays too underpowered. Great game design, wish more shmups made scoring beneficial.

Wrong dumbfuck, they have no accurate slowdown for any sh-3 game it's mostly non existent and due to the hardware architecture will be a complete bitch to implement. Ibara is the best sh-3 emulated titled as slowdown doesn't matter at all aside from maybe milking the final boss archer style and i know no vr pleb is.

Yeah that is correct, slowdown is emulated through a hack called blitter delay or something.
But the deal with Akai Katana is that if you turn on Blitter Delay the sprites will flicker and the bullets disappear so the MAME team is still looking for a way to fix that before officially adding the game to the rom list.

It appears that you do not know about the blitter delay hack:
To turn on blitter delay press Tab, go into Machine Configuratin, turn it on and reset.
There's a bar under Slider Configuration that you can use to control slowdown intensity, the default is 50%, which is about 5% faster than the Xbox360 ports which in turn are 5% bit faster than the Arcade PCB.

Btw you should really check out the recompiler in the latest MAME, it's amazing!

Blitter Delay slowdown is not "arcade accurate" but is good enough for casual play. In theory, if you set blitter delay to 60% it should be closer to the arcade but the sprites will flicker

I have and was playing via metallicx slowpoke mame for years anyway, sh-3 is trash and all the games mostly are to for reason only good players notice, ibara and pink sweets are the only games that have little slowdown on hardware and don't need it anyway so none is bonus, the rest however are really built around it. Cave peaked at ketsui then downhill in both directions into ugly pre renders and terribly implemented slowdown that goes from 10fps to 60fps in a second and trolls you.
