Just finished the whole thing on BloodGDX. Fuckin' amazing. Finally a good source port. Easily the best Build Engine shooter.

Before we get there though, can someone tell me how to run Blood as best I can on dosbox? My specs:

i5 7300hq
Nvidia geforce 1060
8gb ram

I mean I can run gta 5 at 60fps 1080p but for blood on dosbox I barely get 60fps at 640x480 native. Is there anything I can do? Someone please help, I should surely be able to run it much better than this.

I highly recommend getting BloodGDX, but if you really want the DOS version:

Download the latest SVN build of DOSBox here: http://www.emucr.com/search/label/DOSBox.. I suppose you know the basic stuff about mount and imgmount commands and how to put them into autoexec, so I won't go over this. Now, for the important settings:
output=openglnb (it's the fastest)
machine=vesa_nolfb (fixes flickering issues)
memsize=64 (the game can take almost 40MB)
scaler=none (again, it's the fastest)

I'd also recommend these settings, but this stuff is purely cosmetical:
fullscreen=true
fulldouble=true
fullresolution=desktop
windowresolution=original
aspect=true

Apart from this stuff, you should also download bmouse for proper dual axis mouse look and DOS32A for some more performance improvements.

You will still eventually give up and play BloodGDX though, because the input latency is ass in the DOS version, even at a good framerate

Damn GDX is surprisingly faithful from what I've tried of it so far. I wonder why people are always shitting on it on blood threads on /V/? Anyway is it possible to play cryptic passage using it? When I tried to launch it using the User episode feature I got a shit load of texture errors.
