It's complicated.

The first version developed was the PC DOS version, that one is a completely different game using the same assests. It's more like traditional Doom like but it has excellent level design and atmosphere for the kind.
It was the first developed, but not the first released.

When they decided to port the game to Saturn they realized that, at the time, on console it was easier to do 3D than 2D due to dedicated hardware for that versus low processing power.

So they basically made an entirely different game using (mostly) the same assets for the Saturn version. A game which this time, was more adventure oriented and which took into account the full 3D possibilities of the Saturn.
People who think Powerslave console is awesome, often don't realize how awesome it is because they fail to realize that it was being developped at the same time as Quake and before other games which were the fundation for 3D of the era, while it does share many design similarities with Quake and the like.

So the Saturn version was released, then they released the DOS version, and then, a year after they ported the game to PS1.

The PS1 port was handled by a different programmer. If you ask me it plays quite differently and is mostly worse in most aspects.
It's still worth playing because they changed about 3-4 stages of the game.
The whole hidden "team dolls" thing was also re-done, and it's pretty much the only thing I consider better on the PS1 version.

In the PS1 version they also got rid of Bomb Jumping which was absolutely awesome on Saturn.

Then, comes EX, an impirical fan remake of the PS1 version. So it's basically a blind port of what is already a port, and everytime a lot is lost in the translation.

TL;DR Play the Saturn version.

Possibly the best game ever made. Shock 2 is definitely a better horror game but the original seriously blew my balls off when I finally got around to playing it. After I finished it and heard it was getting a remake my first thought was "It doesn't fucking need one, it still holds up great." At least that'll get more casual scrubs playing one of the all-time greats I guess.

Incredible level design, one of my favorite OSTs ever, imaginative weapons, great enemy combat and so goddamn immersive. Every couple of years I replay the game and I always find something new. It's sublime and will always be one of my favorites.

I don't know, I've been mainly playing the PAL version on actual hardware.

FYI the PAL Saturn version was ported to run at the intended speed (the PAL PS1 version however runs slower).
The only difference is that firing rate of some weapons will be a bit lower, but everything else plays at the intended speed.
The PAL version also has some other differences, mainly 2 glitches (1 one which makes that version the best for speedruns) and they took out several HP/AMMO refill items throughout levels, as well as some other minor level design changes.

Also I've been emulating the Saturn NTSC-U and NTSC-J versions just fine using Yabause (0.9.15-win64). The only issue related to emulation was that the background image during loading times wouldn't appear, very minor.

I should still have somewhere isos of all 3 Saturn releases of the game if you can't find them.
Took me months to find a rip of the Jap Saturn version, but sadly it doesn't have as many differences as the PAL version. It's just like the NTSC-U version except there is more ammo for the Cobra Staff and Amun Bomb weapons (which CAN be useful), pic related Jap Saturn version title screen in Yabause
