That's idiotic. It's a fictional depiction of equipment associated with medical personnel - which, IRL, CAN use red cross symbol.

It's one thing to use it as generic symbol for health - but Doom DOES NOT, it's clearly here used in context of medical military personnel (used to indicate that these medics are fucking dead because demons don't respect Geneva convention, apparently). I mean, the fact that throughout Doom you're scavenging equipment of dead Marines is pretty explicit.

And honestly, this design sucks fucking balls - white side of the pill doesn't stick out from the grey of the pack enough, making it appear from a distance as a grey box witu an off-center red dot. They should've made other side of the pill yellow if so inclined, or just go full "arcade shooter" and make it a red heart (which would also work for the black humor angle).

Or just do what MOST people around the world did and make the cross green/blue. Which would also have taken much less effort, ironically.

If you just do homework in private, no. The Red Cross really tends to not care unless it's used in major branding or advertising.
If you made a game about a giant red cross throwing red crosses at people with a big red cross on the cover and called it "red cross simulator 2017" then yeah you're going to get sued, but if you have a medic character or something with a red cross on their uniform or a medkit with a red cross on it, they don't care unless they have a bone to pick.
Devs censoring red crosses out of their games are really only doing it as a borderline paranoid precautionary measure.
