I've heard that Super Mario Bros. 2 jp, AKA: The Lost Levels wasn't initially released outside Japan, because the publisher thought it was too hard.

Is this actually a true fact, or just speculation?

if I remember correctly it also had to do with how outdated the game looked by the time it would have been released in other regions. It was almost the same game as the first one visually and Nintendo thought that it wouldn't sell because of this.

This is correct, also Miyamoto had a special love for Doki Doki Panic and wanted it to reach a wider audience

Simple answer, because SMB is too easy. The US got the best of both worlds with the arcade game Vs. Super Mario Brothers anyway. It has the later, hardened versions of the earlier stages from SMB substituted backwards and the late stages have the best most properly balanced stages fro SMB2J hand selected and arranged.

It's only too hard because the lack of a good save system.

If you play the all stars version, where you can save after each level, the difficulty suddenly vanishes.
