A typical cart has something like 1 mask rom and one or two caps on it. Going by the MX23L1610 specs (a typical 16mbit mask rom which S&K may even use), that thing uses 25mA when operating, less than a thousandth of that when idle. I'm counting about 25 carts on that picture, so that's 625 mA - assuming that every single ROM is in use. The Megadrive power supply is 1.2A, so it's not inconceivable for that thing to run off of that.

However, the actual power usage is much lower, as only the first cart is accessed (barring a one-time check of the 2nd cart). The Megadrive can't access more than 4mbyte so anything past the second cart wouldn't get accessed, and the only time the game itself bothers accessing more than itself is to check if there is anything beyond the 2mbyte area, to see if it has Sonic 2 or 3 locked on.

So the power supply may not be able to power that many carts (even that is up to debate!), but the carts are all idling so they don't even use a significant amount of power.

The biggest challenge in running that tower is holding them down so the carts don't bend outwards, bending the contacts by their sheer weights and causing a disconnect.

and yes, it really did work, I even had a game genie in.
It won't load sonic 1/2/3 unless you remove the game genie though. Then it goes up to 4 S&K carts before the non S&K games won't load.
