I grew up with a Sega Genesis but I feel like there are some games I've probably missed along the way that were exclusive to it. That or I just didn't give them the chance they deserved. I mostly stick to the popular ones like related picture, Sonic, Road Rash, Ristar, and so forth so obscure ones are welcome. The better the overall package the more I think it'd keep my attention (gameplay, music, etc.).

Some slightly different recommendations:

Star Control - 2 player asteroids-like with a range of different ships that have different "personalities" and special moves, like a 2P beat em up

Starflight - kind of like a 2D Elite with space exploration, mining (I fucking love mining !!) alien negotiation dialogue, space combat, and an awesome storyline

Buck Rodgers - A D&D style top down turn based space RPG and a pretty good one if you like that kind of thing.

Herzog Zwei - Obscure early Genesis title. Could be the father of real-time strategy, you command a giant mech and set factories to create tanks, troops etc which you issue command to. Great music

Truxton (Tatsujin) - No frills vertically scrolling shooter that does the basics really well. Also great music - really low rez late 80's arcade synth
