Super Mario World can only handle four turning blocks at the same time. Spinning with the cape into a whole bunch of them can flip more than that, causing the first block to spontaneously rematerialize to make room for the fifth. If that first block is on top of Mario, the game tries to push him to the nearest open space--potentially through walls.

Under the hood, Super Mario World is a bit of a mess.

Well since it was the first SNES I'm guessing they just did their best with programing. On a normal playthrough you should see hardly any glitches/bugs. I can see OP glitch happening to someone having fun spinning around in the cap surrounded by those blocks but outside that I can't really see most people facing any problems. It's not poorly designed it's just has issues that you don't really notice until you actually open her up and see what makes her tick.

IIRC it's done by confusing the game into calling an end-of-level/game "object" of sorts, through programming errors, memory corruption, etc. I have no idea that's just what I've read.
