My uncle used to be a level designer and artist for Nintendo back between 1992 and 2001 before he was deported for stealing 2 cans of coca cola from a 7/11
He told me Letha lava land was called motos buttplug factory and the lava was cum and that's why the platforms are like conveyor belts. Some of the platforms were grass so they put a flower effect on there. The level was scrapped before they finished the stage fully, them meaning to add assets that would later become dick docking clock which was renamed to tick tock clock.
