It's very different from basic Doom, because the shooting mechanics are much more like realistic shooters. Probably also has some of the most realistic and detailed wounding mechanics of the entire genre of first person shooters, in how wounds need to be properly managed, and injures stack up. Of course, it's still Doom as a setting, so there's ridiculous sci-fi stuff to keep you going, but even if you get fucked up beyond belief to the point medical science couldn't fix you, it's again, still Doom, so besides demons from hell, and hell itself, there's also magic, and the right kind of magic can fix just about anything. The wrong kind of magic can also ruin things.

What makes it stand out from other realistic shooters is that it's Doom, so you still have the abstract level design, you still have the aforementioned demons, and their vicious magic, so besides dudes with guns, there's reinterpretations of all the classic monstrosities to be truly merciless in their own ways.
The monsters don't play very fair, and given the nature of Doom levels being made for a very different kind of gameplay, some levels aren't necessarily realistically beatable, or they require unorthodox approaches, including breaching locked doors and barriers with explosives, shooting through walls, setting tripwire grenade traps, setting up different kinds of sentry guns, remote activating doors and switches, etc, so the player has all kinds of tricks on their own to use against the monsters.
With how item spawning is randomized, and you have an inventory to keep stuff in, Hideous Destructor also has a strong Roguelike element to it, you can ransack a random ammo box or backpack to find something you didn't know you wanted.

It has a learning curve, and isn't for everyone, but it's also a very unique flavor. You can be very slow, methodical, and cautious in your approach, but often being fast, reckless, and hardboiled, can give you a the best results.

If you really want to up the challenge, there's Hideous Destructor ("Tyyyyyrant"), which, to describe it is kind of like turning Doom into a mix between ARMA and SWAT 4, with a heavy emphasis on realism (as far as Doom goes), except instead of just fighting dudes with guns, you're also fighting supernatural demons, and the approach is that to be realistic threats, all demons should actually be really dangerous and really troublesome.

It has a bit of a learning curve, and some levels are not realistically playable with the mod (though Doom's original three episodes actually play pretty well with it), but some find it quite enjoyable, because the combination of realistic gunplay and wound management, with evil magic and demons, can't really be found anywhere else.

One of the quirks of HD is that not every level out there will be realistically beatable, or only on some specific ifs. That said, the original three episodes for Doom, E1, E2, and E3, are actually quite playable, and would be a good place for someone interested in HD to start (aside from maybe doing the bootcamp first).

It's Doom if the gameplay was closer to games like SWAT 4, Arma, etc. It's probably the most realistic gameplay mod for Doom there is, in how you manage weapons and inventory, handle wounds and injury, etc, but one of the core parts is that the demons are made really dangerous and bothersome,
so that a group of them could realistically fight and win against a group of modern soldiers. Though the monsters may feel like they are unfair to you, the gameplay also encourages you to be unfair to them just as well, by using certain weapons and items to destroy some locked doors, or by climbing and reaching places you shouldn't to potentially bypass certain sections and ambushes, and penetrating certain wall surfaces, depending on their thickness and your gun.

It takes the tactical realistic shooter gameplay and puts it where you have to fight with supernatural demons from hell and still navigate Doom's abstract kind of level design. Though it has the most realistic wound mechanics of any Doom mod, you can still get away with all kinds of bullshit with sci-fi healing items, magic, and Berserk packs. The difficulty is also fairly adjustable with options.

It's very much not for everyone, and it's an acquired taste, if you don't like really realistic shooters, you probably won't enjoy Hideous Destructor, but it occupies a pretty unique niche for first person shooters, which is why it has been growing popular.

at this point, between ladders, wall destruction and penetration, and various added mobility features, the player really has the means to survive on just about any map. there's no need for specialized maps made for the mod anymore. just play doom 1 stuff for a softer introduction, and move onto easier doom 2 wads when you're ready to learn those enemies. in time, you'll be able to handle the likes of scythe, alien vendetta, and valiant.

the fire mechanics are nasty but instances where you have to rip your armor off are greatly reduced, since regular imp fireballs just hit you with some "sticky fire" kind of shit that doesn't come off with your armor, but doesn't light you up either. this means they're way more dangerous than they used to be, because now instead of taking your armor off as soon as you're hit and losing as little as 1 or 2 hp, you'll always take around 20 for each fireball that hits you. luckily they don't have the range and aim that they used to.
