It's definitely set on Mars. Other places are not announced yet, but they have confirmed that you will not go to earth.
They do mention that the environments are going to be a lot more varied than in Doom 3, however this could mean anything.
Who knows? Maybe we will get flamboyant space colonies and a brilliantly fantastical Hell.

Hell should feel like a place where the demons really do live and have developed a culture. It should show that they are savage, yet intelligent. Doom 64 had some of the best hell levels because of this.
I wouldn't mind a paradise lost inspired back story. However the fallen angels have all sacrificed their flesh in order to create the demon army. The ultimate goal is to conquer the moral world as vengeance for their downfall. but then again I am a faggot who wants a deep story with HL styled story telling

Gameranger works that way by launching the games, but the background issue I think has to deal with game versions, since it often has happened to me when trying to connect to people there. i assume you guys looked into versions already, but just for safe measures you could try torrenting/buying from the same source.
This type of problem is either game version or some sort of internet blocking, a la firewall or whatever. Try turning off your firewalls when you're connecting to each other (then turn them back on if youre feeling unsafe).
there also might be a small chance that one of you guys have a 'port-restricted cone nat router', which could be preventing those with symmetric routers to connect. if you did some port forwarding you might've already fixed this, but make sure your router is portwarding UDP port 16000
if all else fails check if your router supports UPNP (universal plug 'n' play) and enable it if it does
