I've seen the last one you mentioned and possibly the first but it was neither
just an elevator dropping you in the center of a room with no way back up and a ton of locked doors
only one of the doors had an exit and every other had an enemy plus maybe some loot

can't remember its name though

I did also see this dungeon that led up to a Giant's thrown room of sorts
at least I thought of it like that because the dungeon was full of giants up in the mountains and there was this huge throne room at the end with a single giant inside
