The way emulators are developed isn't the chief reason for inaccuracy. It takes time & effort, but "perfect" accuracy is possible for any system, and simpler systems like NES are 100% understood.

Lots of divergence from original hardware is seen as beneficial, like perspective-correct texture mapping on PSX emulators. There's no aesthetic benefit to affine mapping and it would require extra implementation effort, so its absence is understandable, even if accuracy is hampered.

Then there's the (I believe) information-theoretical impossibility of perfect emulation of analogue DACs and synthesizers and such. Any analogue signal processor can only be approximated, not replicated, by a digital one. Still, things can reach the point where most people couldn't tell the difference.
