The music and visuals are stellar. Seeing graphics like that just fly out of the screen was super impressive (same thing applies to Space Harrier, really). It's a really pick-up-and-go game -- no matter what, every run will take 5 minutes or less.
It continues to be interesting due to time attack, the branching paths, and dealing with the traffic.
There's a lot to love. Even the Genesis version, despite being fairly cutback in terms of framerate (and I'm pretty sure it could have been better for a Genesis release in 1991) is fairly faithful (especially compared to many of the other ports, just about all the other ports without realtime scaling fuck up on road width and split roads, which the Genesis version does just fine).

I like the OutRun port on Genesis. Totally competent.
it really could have been better, but it's a semi-early title on the machine (I can't think of anything terribly impressive graphically on the machine from before 1992) and it gets the job done
also, which romset is the Genesis version even based on?
the stage order is different, there's supposed to be the one with the arches as the left second stage

man, I still think about a world where the Genesis was released in '90/'91 instead of '88/89, where it might have had some non-zero chance of getting some scaling hardware in there
like, most of Sega's big mid/late-80s arcade hits NEEDED scaling of some kind

2019 is definitely a solid game though. Other than it being really drab looking, I dunno why people seem to dislike it so much.
wasn't originally going to be an OutRun game apparently, it was supposed to be some Sega CD game called Cyber Road or something
