Not possible, and the included DOSBox Version is not supporting separate X and Y mouse sensitivity setup. You'll have to update DOSBox with one that comes with this support and modify ROTT.conf sensitivity to 100,0 or you try a source port. I'm using this: https://dosboxece.yesterplay.net/

Alternatively, you can download a program called novert.com from the idgames archive. launch that program in dosbox, then launch ROTT from that same window. for example:
>mount d: d:\
>d:
>cd dosgames
>novert
>cd rott
>rott
Also you should download the DOSBox pixel perfect fork and use that. Not required but recommended.

Extreme Rise of the Triad is DEFINITELY not for everyone and there are some stage ou literally cannot beat with the male characters because they're too slow. There were some secrets in the original that were like that but this is to just get through the stage. One in particular puzzle (pushing pillars into a row and running across the top of them) can ONLY be done by Thi Barrett for some reason even though Lorelei is faster. Also the platforming gets insane in a game without a jump button, clearing huge gaps from GAD to GAD 10 stories up.

Taradino Cassatt:
Mr.Average. That's why he's in the shareware episode. He
has average life, average speed, and average weapons accuracy. If nothing
special is called for, Cassatt's a good choice.
Thi Barrett:
She's pretty average, too. She's a female average character, in case
you don't like Cassatt or you prefer a female character. She's a little
faster than Cassatt. My personal favorite.
Doug Wendt:
This guy is a tank. He's pretty slow, and only has average weapons
accuracy, but, man, he can take tons of damage. Lots o' hit points here.
Lorelei Ni:
Not many hit points. Fast. Extremely good weapons accuracy. Not
for beginners.
Ian Paul Freeley:
Freeley is a player who is provided for a happy medium between players
like Cassatt or Barrett and Wendt. He's got more hit points than Cassatt
or Barrett, but he's slower. On the other hand, he doesn't have as many
hit points as Wendt, but he's faster.
