I don't know how they're "supposed" to work really. I just created two custom resolutions of 2560x240@120hz and 2560x480@120hz for my CRT in nvidia control panel, and Retroarch seemed to switch to those resolutions just fine after entering them manually and the games looked great.
It wouldn't switch between the two automatically for 480p consoles though, and Retroarch wouldn't reset the resolution after quitting for whatever reason which was quite annoying.

Here's a pic for you. IBM G72, not impressive for modern stuff as it has a max res of 1280x1024@60hz, but good for 1024x768 and lower. Thinking of putting it into a DIY arcade cab some day.
