Base game? No, unless we're including multiplayer.
With mods? Yeah, a lot of Quake 1 custom maps and mods improve Q1 enough to put it on a similar level to Doom.

I think you can edit the sounds in the .GRP if it bothers you that much. Otherwise, apply Devastator/Shrinker to it's forehead.

I'd argue Quake as a brand is muddled up in general due to the weird spread of mechanical body horror and Lovecraftian themes between games, so overall people who've heard of Quake these days associate it more with multiplayer than a particular theme or story.

Although if I were to attempt to successfully reboot the series I'd base it on Quake 1's themes due to it being far more distinguished than giant sci-fi tech bases.

I think the most subtle but greatest difference that AD does different from vanilla Q1 is that how the encounters actually match how fast Quake 1 plays with a mouse, so it ends up being a lot more engaging to play.
(Oh, and the mapping porn too.)
