They have a team, there are the guys who have the "vision" of the level and detail the basic layout, then there are the other guys who add all the extra little details.
This allows moments where everybody too can feel like they're adding their own touches, for example a vision guy just imagined a hallway and then a corner, but a details guy might add blinking lights and a little diorama or something.

This allows for more detail, while simultaneously reducing workloads overall. Especially too since there are some people who prefer the "big picture" stuff and others who love details and both dislike doing the opposite.

There was an anon I saw posting here years ago who just loved doing minor details on things, nothing pleased him more than detailing tiny little lights and floor patterns and he lamented he couldn't team up with somebody who would be a "big picture" designer.

As somebody who can lazily draw a whole level concept during a train ride to work, I'd love to team up with somebody like that. As detailing corridors makes me want to blow my head off.
