I'll take a wild shot. Is it Kid Chameleon? It was a side scroller and you could get a bunch of different helmets that made you other characters (depicted). I remember there being a single automatic scrolling level with a spike wall behind you... as a kid I went the wrong way and I swear a voice shouted "DIE!" at that moment. Never played again because I was an easily freaked out little shit.
