Are you using HD mod? Anyway, you can't have perfect scaling with a lower res with 1080p because the game's original res is 600p, which is more than half of 1080p.

Basically here's how to achieve pixel perfect, unaltered, sharp upscaling:

1.You need a monitor with vertical resolution of AT LEAST 1200 pixels
2.Select 32-bit (stretchable) GDI as your graphics mode
3.Choose any resolution that's exactly your monitor's native res divided by a whole number AND that has a vertical resolution of AT LEAST 600 pixels.
4. Select Nearest as your stretch filter.
