Adventure Island doesn't have as many colors but still looks pretty good, and IMO just the fact that the music isn't repetitive shit and actually offers a catchy and good sounding soundtrack makes it the better game by far.
I can't remember if there was any other significant change in gameplay or design, I haven't played Wonder Boy since the 90s probably, but to me it played exactly like Adventure Island on NES.
The Adventure Island sequels are much better anywho.
