The Loot Box mod kind of missed the underlying point of Loot Boxes; they're the end-result of modern games moving entirely to an XP-driven progression format.

A DOOM mod that replaced health with experience points and required you to grind enough experience from pickups / kills to unlock the next level might actually be somewhat interesting. Loot Boxes could be bought at the end of a level at the cost of valuable XP for the hope of some boosts for the next level.
