Are you playing the unpatched .wad? The yellow key is accidentally tagged not to spawn in singleplayer. Team TNT actually caught this and submitted an update to iD many months before Final Doom was published, but for whatever reason they never added it.

Your options are to either cheat (because let's face it, the map is broken), do a speedrunner maneuver to get on top of that throne, or to just close the game, apply the patch, and start the level over.
