For awhile I was doing nightmare, and I was only getting about 100k-200k per run, whereas in hell it's closer to 150k-910k in one run. Certainly you can do about 3-4 nm runs per hell run for time.

I think you're right I should focus on gold, but I get the skinner-box dopamine hit when the late game uniques drop and the high runes, haven't found any godly rares yet though.

The BOTD war pike damage is massive, i'm assuming you were using the act 2 merc with might aura, i wonder if the 45% CB on the spear makes that huge of a difference, because that BOTD has double the damage of hone sundane + you had the ED and res from fort. it's possible you had a consecutive string of really strong monster modifiers when you tried to clear the packs in hell
